Our Mission
Community Health Connection is a Federally Qualified Health Center (FQHC) committed to meeting the lifelong health care needs of the northeast Oklahoma community. We provide the highest quality, affordable and accessible primary health care services in a culturally effective, language-appropriate and compassionate manner. Voted one of the "Best Places to Work" in Oklahoma for the past five years.
We utilize a Primary Care Behavioral Health Model- Utilizing an integrative approach to care to address multiple patient care needs from a multidisciplinary team perspective while implementing integrated behavioral health services. We consider integrating behavioral health services in our primary care settings an innovative and necessary service model targeting patients who typically have limited access to traditional mental health treatment. Our BH team works collaboratively with the medical team to implement a system of comprehensive screening, direct patient care, and consultation with the multidisciplinary team to improve overall health outcomes and increase access to mental health care for our patients. In addition to providing evidence-based treatment for traditional mental health issues our BH team helps identify and address many behaviorally driven health concerns that arise during medical care. Consistent with an integrative care model, our BH team works to strengthen collaboration with other service providers in the community to create a more effective system of referrals for patients with ongoing mental health needs. Our BH team holds a key position in helping the organization fulfill its mission of providing the highest quality care for vulnerable and underserved populations.
